Five document questions for youth-service families

Understand the purpose of service-scope descriptions, intake instructions, credential claims, communication procedures and privacy notices.

Choose the document question you need to answer. This library is organized around reading and clarification, rather than around selecting a treatment program or arranging travel. The useful result is a clear description of what a document says and the question to bring back to its issuer.

What is the service's stated scope?

Locate the written description of included tasks, separately arranged services and the roles responsible for each. Ask who provides clinical assessment or treatment when that work is part of the proposed plan. Keep the provider's actual language in your notes before adding your own interpretation.

What do the intake instructions request?

Identify the requested item, the reason given, the receiving channel and any deadline supplied by the provider. Ask about an unclear request before sending private records. Keep a record of what the organization asked for, rather than assuming every document in an earlier packet is needed again.

What does the credential claim refer to?

Record the person's name, precise credential, issuing authority and relevant location. Ask which person will perform the described role. SAMHSA identifies licensing and accreditation as important treatment-quality questions; this library helps readers prepare that question without awarding a credential itself.

How does communication work?

Find the procedure for routine updates, administrative questions, records questions and urgent concerns. Ask the provider to identify the responsible role and actual channel for each. Family participation should be discussed with the person receiving care and the responsible professionals, using the arrangement that applies to that service.

Which notice explains information practices?

HHS describes privacy notices and publishes model notices, including revisions reviewed in February 2026. Locate the organization's current notice and its named contact. Ask which notice applies and how to obtain an explanation of an unclear passage. The questions in this library do not determine which legal framework applies to a particular provider or case.
